Angie’s Brookside Diner, 1360 Eisenhower Blvd. Harrisburg, Complaint, May 17. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Food Employee observed donning single use gloves without a prior hand wash. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Observed food (fried potatoes, chopped vegetables?) in the three-door freezer unit area without a cover. Observed food (mixes?) in the three-door cooling unit area stored open with no covering. Observed a cell phone stored inside a stain steel bowl in the kitchen area. Observed soda gun and holster pink and brown slime on interiors in the bar area. Unwrapped and unprotected single use straws stored in the customer area. Soap was not available at the handwash sink in the bar area.

Laki Fried Chicken,4404 Oakhurst Blvd. Harrisburg, Opening, May 16. Pass. Employee restroom needs signage reminding employees to wash their hands before leaving the restroom.

Pho Miss Siagon,1736 E. Chocolate Ave. Hershey, May 16. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed raw meats stored above ready to eat foods in freezer in the outside storage room with potential for cross-contamination. Food utensils in cooking area observed stored in a container of water temped at 75°F and not at 135°F or above as required. Observed single-use containers stored in pitchers of flour and sugar without handle that could be stored above the top of the food. Corrected on site. Observed wet wiping cloths stored on prep table and on cooking equipment in the kitchen area and not stored in sanitizer solution. Observed bain-marie unit in food prep area of the kitchen holding numerous foods (shrimp, chicken, fish… etc.) at 60°F and not at 41°F or below as required. All food items being held in this refrigeration unit was disposed of on site by person in charge. Loose/deteriorated rubber door gaskets observed on the refrigerator doors of bain-marie unit in the kitchen area and are in need of replacement. Observed the following on food contact surfaces: Accumulation of old food debris on tinfoil used for shelf coverings on small storage rack near the kitchen back door storing food ingredient bottles and is in need of more frequent cleaning or replacement; Food ingredient bottles throughout the kitchen area with old, encrusted food residue around nozzles and exterior and are in need of more frequent cleaning; Pitchers storing flour/sugar on shelf underneath prep table with accumulation of dirt/food debris on the lids and exterior surfaces and are in need of more frequent cleaning. Observed cooking equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces and is in need of more frequent cleaning. Observed old, dirty cardboard being used as shelf coverings in the kitchen area which is not an easily cleanable surface and should be removed/discarded. Observed kitchen screen door to the outside being propped open and cannot close properly resulting in gaps that could potentially allow rodents/insects to enter the facility and is in need of repair. Observed large number of bees flying around overhang roof above entrance to the outside storage room indicating evidence of a nest in the structure with potential for bees to enter the facility. Observed employee cellphone stored directly on cutting board actively being used to cut vegetables on prep table in kitchen area with potential to contaminate. Corrected on site.

Shangri-La Restaurant & Bar LLC,408 Mountain Rd. Harrisburg, May 16. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Observed cell phones in the prep-kitchen area and the sushi area. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Observed innumerous type of food sauces… in all cooler unit stored open with no covering. Observed 10 containers of assorted foods on a three shelf cart in the kitchen area stored open with no covering. Observed food ingredient storage containers and bottles, through-out the facility, is not labeled with the common name of the food. Observed wet wiping cloths in kitchen area, not being stored in sanitizer solution. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in all refrigeration units, is not being date marked. Commercially processed refrigerated, ready to eat, time/temperature control for safety food, located in all refrigeration units, and held more than 24 hours, is not being marked with the date it was opened. Cardboard box was observed that were reused to store cloth napkins. Sponges observed in the bar area being used to clean food contact surfaces. Thermometers for ensuring proper temperatures of food are not available or readily accessible. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. Observed the following on food contact surfaces: Ice scoop observed stored on top of ice machine, which is not a clean surface, and contaminating the scoop; Interior of a microwaves, with accumulation of food debris on the interiors and not clean to sight or touch. The exteriors of multiple food containers with accumulation of dried food debris on the exteriors and is in need of cleaning. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed a box with cloth napkins stored in the dining area directly on the floor, and not 6 inches above the floor. The handwash sink in the Sushi area was blocked by a package of fish and not accessible at all times for employee use. Exit door located in the back area of the food facility has a gap in the bottom and does not protect against the entry of insects, rodents, and other animals.

Holy Name of Jesus School,6190 Allentown Blvd. Harrisburg, May 15. Pass. Observed Upright food warmer located within the kitchen area, has an accumulation of dirt and old food residue and in need of cleaning.

Hornings Country Store Llc,5904 Rte 209 Lykens, May 15. Pass. Prepackaged food is not labeled with the ingredients, including any sub-ingredients. Some of the products included packaged cheeses (refrigerated) and chocolate/caramel coconut candies in the back storage area, and sprinkles/colored sugars stored throughout the facility. Observed single-service, single-use containers in the back storage area stored directly on the floor, and not 6 inches above the floor. Food facility has not retained the most recent sample report for the non-public water system on file, as required. Receiving door located in the back of the food facility has a gap on the bottom of both sides of the door and does not protect against the entry of insects, rodents, and other animals and is in need of repair. Observed too many to count rodent-like droppings on the floor between chest freezer and racks in the back storage area.

The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Observed numerous food items (General Tso, egg rolls, chopped vegetables, raw frozen meats, etc.) in the walk-in cooler, two white chest freezers and baine marie cooling unit on the cook line area stored open with no covering. Observed no food ingredient storage containers/bottles/shakers, in the entire kitchen area, including dry areas, counters and cooling/freezing units, is not labeled with the common name of the food. Food dispensing utensils in numerous food spices/ingredients observed stored in the food and not with handle above the top of the food and the container or utensils (single use) with no handle touching food. Observed numerous food items (commercially processed and food prepared in the food facility), (sauces, fresh cut vegetables, noodles, egg rolls, etc.) in the walk-in cooler and Precision baine marie cooling unit, and being held for more than 24 hours, is not dated marked with the date it was opened or made. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in cooling/freezing equipment. Bleach used for sanitizing in the three bay sink is not approved for food contact surfaces. Observed walk-in cooler leaking into two gray bus tubs and in need of repair. Observed white Frigidaire single door freezer with accumulation of ice build-up on the interior and in need of thawing. Observed Kenmore chest freezer with broken lid and in need of replacement/repair. Observed second chest freezer, with missing handle and no name of unit available, with a broken lid and handle and in need of replacement/repair. Observed the following on food contact surfaces and not clean to sight or touch: Two magnetic knife holders over the three bay sink area with dried food debris on the knife holders coming in contact with numerous knives, cleavers and strainers stored. Interior of the Black & Decker toaster oven with heavy accumulation of charred food. Interior of Panasonic microwave with used dirty papers and food debris on the entire interior of the unit. Numerous spice/ingredient storage containers/bottles at the cook line with ingredients that are not being changed out daily and heavy clumping, dripping, etc. on the food ingredients stored. Ticket machine with heavy grease and food debris accumulation on the exterior contaminating persons taking tickets, even with a gloved hand. Numerous utensils, at the wok cook station placed directly on the wet and dirty non-cook area around the woks contaminating utensils. Precision 2 door bain marie food ingredients containers are not being changed out daily, nor are utensils being removed to be cleaned daily. Large lid to the rice cooker stored dirty and leaning against the dirty hot water heater in the kitchen area. Observed interior and exterior of white GE standard refrigerator on the cook line with heavy accumulation of food debris and staining on the interior and exterior of the unit and in need of cleaning. Exterior of Black & Decker toaster oven with accumulation of burnt and dried encrusted food debris. Exterior of Panasonic microwave oven with accumulation of food debris and grease on the exterior. Exterior handles on all food storage equipment (cooling/freezing units). Ingredient bottles, spices, sauces stored under the stainless table at the end of the Precision two door baine marie with heavy food build-up on the exterior of containers with the possibility to contaminate foods and hands touching containers. Single burner side of the wok station with heavy accumulation of grease build-up and not clean to sight or touch. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed facility to be very dirty and disorganized with food contact and non-food contact items stored with chemicals and food debris in various areas, including the flooring, ceilings, restrooms, kitchen, etc. areas. Observed the hand-wash sinks (2) in the restroom area with no water/no hot water for hand-washing. Observed broken urinal in the customer/employee restroom area, and in need of repair/replacement. Soap dispenser was empty at the hand wash sink in the kitchen area and the restroom area. Kitchen area was corrected on sight. Old unused equipment stored in various areas of the facility, should be removed from food facility.

Observed numerous food ingredient storage containers, bottles, shakers, in the entire facility area, is not labeled with the common name of the food. Observed numerous foods, sauces, etc. was held at 55 °F, in the Blue Air Baine Marie Cooling unit, rather than 41°F or below as required. Owner had employee remove all food and place in large walk-in cooler until unit is repaired. Observed puddling of water on the exterior of the large walk-in freezer and is in need of repair.

Pizza Hut,675 Main St. Hummelstown, Type 2 Follow-up, May 15. Pass. Observed the following on non-food contact surfaces: The area behind the three bay sink with heavy brown/black mold-like substance under the signage and grease on the racks above and is not clean to sight or touch. Exteriors of all cooling, freezing and warming units handles are in need of cleaning. Ticket/order machine with dried food build-up and grease which could cause contamination even on a gloved hand when reaching for order tickets. Interior doors of the Randell Baine Marie Pizza unit with a black mold-like substance on the interior gaskets, door jams and interior doors. An accumulation of static dust on the ceiling light fixtures, ceiling tiles (especially in the prep/cook line)of the entire back area.

Anafe Restaurant,1222 Mulberry St. Harrisburg, Complaint, May 14. Fail. NO person in charge present. Observed cooking equipment/pans, in the kitchen area, with encrusted grease and soil accumulation. Observed hood system equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Floor/wall/ceiling, in the bar/counter area, is made of plywood materials, and is not durable, smooth, non-porous, non-absorbent. Paper towel dispenser empty at all the handwash sinks in the facility. Soap was not available at the handwash sink in the bathroom and cold hold areas. No sign or poster posted at the handwash sink in the area to remind food employees to wash their hands.
